About the rewards

  • How do I earn rewards?

    By attending your scheduled appointments! You earn $15 for each MAT medical visit, $15 for each counseling or mental health visit, and a one-time $25 reward for completing your Hepatitis C screening.

  • How do I get my gift card?

    Our staff confirms your visit when you come in, and you receive your Walmart or Kroger gift card right at the clinic. Questions about a specific card? Ask at the front desk or call (606) 225-8200.

  • When do I get my reward?

    At the clinic, at the time of your attended visit, once staff confirms you were there.

  • Is there a limit to what I can earn?

    Yes, the program cap is $750 per person over the 12 weeks.

  • What can I buy with the gift card?

    Most everyday items at the store. The cards cannot be used for alcohol, tobacco, firearms, or lottery purchases.

  • What happens if I miss an appointment?

    Nothing bad! You stay in the program. You just do not earn a reward for that visit. Come to your next appointment and keep earning. If you need to reschedule, call (606) 225-8200.

  • I lost my gift card. Can I get a new one?

    Unfortunately lost or stolen gift cards cannot be replaced, so keep them somewhere safe.

  • Why does this program exist?

    Because showing up is one of the most powerful things you can do for your recovery, and we want to celebrate it. This program is made possible through a SAMHSA-funded pilot project administered through the Kentucky Opioid Response Effort (KORE) and Fresh Start Health Foundation.

  • Is this a research study?

    This rewards program is part of a SAMHSA-funded pilot administered through Kentucky KORE. Your care team is happy to answer questions about what participating means for you.

  • Do rewards affect my treatment?

    Not at all. Your care is exactly the same whether or not you take part, and you can stop participating at any time without penalty.

  • What services does Fresh Start offer?

    Addiction care (including Suboxone, Sublocade, Brixadi, and Vivitrol), counseling and group therapy, psychiatric care, primary care, Hepatitis C testing and treatment, PrEP and STI services, case management, peer support, telehealth, and free transportation to appointments.

  • How do I make or change an appointment?

    Tap Request an appointment on the Schedule tab in this app, call us at (606) 225-8200, or use the healow patient portal from freshstarthealth.com. Same-day and next-day appointments are often available.

  • Where are you located?

    Four eastern Kentucky locations: Ashland (336 29th St., Ste. 203), Grayson (207 N. Carol Malone Blvd, Ste. 3), Vanceburg (207 Plummers Lane), Morehead (150 Newtowne Square). One number reaches them all: (606) 225-8200.

  • Do you offer rides to appointments?

    Yes! Fresh Start offers free transportation for patients who need help getting to their visits. Call (606) 225-8200 to arrange a ride.

  • What insurance do you take?

    Kentucky Medicaid and Medicaid HMO plans, Medicare, Aetna, Anthem, CareSource, Humana, Molina, UnitedHealthcare, WellCare. If you are unsure about coverage, call Fresh Start's office at (606) 225-8200 and the team will help you figure it out.

  • Can I talk to my doctor or nurse through this app?

    No, this app is not a way to reach your care team. For anything about your health or medications, call (606) 225-8200. If it is an emergency, call 911.

  • What if I am in crisis right now?

    If this is an emergency, call 911 right away. If you are struggling or having thoughts of harming yourself, call or text 988, the Suicide and Crisis Lifeline, any time, day or night. You matter, and help is available.

About the app

  • What do I see on my home screen?

    A thought for today, your next scheduled appointment, your attendance streak, and the rewards you have earned so far. The Schedule tab shows every visit, and the Rewards tab lists each reward.

  • Who can see my information in this app?

    Only you and the Fresh Start team members who run the program. Your app activity is protected and is never shared publicly.

  • How do I follow Fresh Start on social media?

    Tap the links in the Help screen to find us on Facebook and Instagram. We would love to have you share your wins with us!
